PROBE the HErD is a guidebook written by Energy Kristina where she provides professionals within regulated industries (i.e., pharma, med devices, food, warehousing/transport) with a method to help reveal and identify the true root cause(s) within human error related deviations, concluding in the reduction of recurrence.

Finding the driving force behind a human error related deviation (HErD) is done through the carrying out of a deep and thorough investigation, called a PROBE. A probe comes equipped with a method and proper tool to help reveal and identify the true root cause of the error at hand:

The 3BMethod is that amazing guidance system which will aid the user in finding the correct place to start probing

Whereas the 3BM Prediction Tree is the tool used for further discovering the true root cause of the HErD.

These two together create a behavioral root cause analysis tool that can be used to strengthen your already established RCA tools that are being applied (e.g., 5XWhy, Fishbone) during investigations.

This lesson is 70% theoretical and 30% practical
